Manager/Supervisor Risk Assessment Toolkit

Our role is to develop and assist in the implementation of the UWA safety, health and wellbeing programs in order to minimise the risk of injury, illness and property damage.

We provide consultancy and other services to promote best practice and legislative compliance in all University and related activities.

The below list of templates have been developed to assist all Managers/Supervisors and Staff across UWA in being able to consistently:

Dangerous Goods (DG) and Hazardous Substances (HS) Risk Assessment

  • A targeted risk assessment designed for use with chemicals, goods or substances in the workplace to identify if they are categorised as dangerous or hazardous as per their related Safety Data Sheet (SDS).

  • Assessment for plant and complex equipment used within the workplace that requires specific controls.
  • Distinct from SOP in that they relate to the whole workspace environment not just the specific machine.

  • Assessment to be used BEFORE purchasing or acquiring and installing new plant and equipment within your workplace.

Root Cause Analysis (RCA) Incident Investigation Report Template

  • A template used to assist Local Safety Officers undertake a thorough incident investigation for High and Extreme related risks and Lost Time Injuries (LTI) to identify: a timeline of key events, contributing factors, root causes and key corrective actions to minimise incident recurrence.

Safe Work Method Statements

  • SWMS's document the processes, licences, and actions required to complete a task safely.
  • They identify hazards within an activity and will provide guidance to workers on how these hazards can be mitigated.

Standard Operating Procedures (SOP)

  • Act as a risk assessment by addressing the specific hazards of the equipment and controls to make safe.
  • Can be used as a record of induction that an individual has been assessed as competent in use of equipment.
